"Northern Electric/ MADE IN CANADA" cast in raised print on phone body casing front. "34" marked on green dymotape label fixed to casing back. "GR5 7761-/ LOC 21 ext." marked on label on dial. Dial faceplate marked 1 through 0, a through P, and R through Y, and "OPERATOR".

Finish: SMOOTH BLACK MOLDED CASING AND HANDSET. BLACK METAL DIAL. SILVER METAL BACKPLATE. WHITE DIAL FACE HAS RED AND BLACK MARKINGS. BROWN LEATHER PADS.
