- Markings
- "George Freeland/ EIGHTY-NINE/ BLOOR WEST/ TORONTO" printed on front cover of paper mount. "George Freeland" printed on insert mount, below photo. "Dr Peperdene [sic]/ Radiologis [sic]/ may [sic] first Teacher" handwritten in black lead pencil, also on insert. UHN catalogue no. "997.4.7" printed in black lead pencil in lower right corner of folder back.

- Black & white photographic print, mounted inside light grey heavy paper folder: folder and paper insert edges are cream-colour.
- Decoration
- Linear border printed on front cover designed to suggest depth. All cover and insert edges have lightly frayed finish. Printed markings are lightly embossed.
