- Mfr's label sewn inside suit, at neckline reads: " MINE SAFETY APPLIANCES CO./ PITTSBURGH, PENNA./ MADE IN USA ". " BuD " handwritten in ink on suit front, below faceplate.
- Missing
- NONE. From CA of 06/29/1998 by Myles Bailey: No - It is a one piece suit with a steel helmet inside and there is a blue glass lens in the hood. All appears to be present.
